Frequently asked questions
- Why lead with clinical risk surface instead of funding stage?
- Because it decides how long everything takes. A founder with no clinical claim can ship a feature on Friday. A founder with a 510(k) in flight cannot change the intended-use statement without restarting a submission, and one running a CLIA lab has a state-by-state permit map to maintain. Two seed-stage healthtech companies on opposite ends of that column have almost nothing in common operationally, and stage alone would put them in the same bucket.
- How many of these are actually selling into hospitals?
- 112 have a health system or provider budget as their primary payer. That is the hardest of the five channels — a security review, a clinical governance sign-off and often a value analysis committee stand between a warm intro and a signature, which is why the 61 selling through employers and the 57 selling cash-pay tend to show revenue much earlier at the same headcount.
- Does the clinician-founder split matter for outreach?
- It changes what lands. The 109 clinician founders generally know the workflow better than any vendor pitching them and respond badly to being told how care works. The 96 with no health background are usually hunting for exactly that context and will take a call that offers it. Same product, opposite opening line.
- What counts as a paying customer here?
- An executed contract with money attached — 48% of the list. Unpaid pilots, letters of intent and innovation-centre engagements are excluded, because in this sector those are common enough that counting them would make the number meaningless. It is a deliberately conservative read of traction.
- Are prescription digital therapeutics still a real category?
- 41 founders here are building one, which is fewer than the same cut would have shown three years ago. The reimbursement path never fully arrived, so several of these companies have pivoted the commercial model toward pharma-funded or employer-funded distribution while keeping the cleared product. Worth knowing before pitching them on anything that assumes payer coverage.
- Can I filter to founders who are not blocked by procurement?
- Yes — the cash-pay and employer channels are the closest thing. Those 118 founders control their own go-to-market and can buy tooling on a card, whereas a founder whose revenue depends on a health system contract is often mirroring their customer's procurement discipline internally, and will route your invoice through the same review.
